Lääketieteelliset kriteerit
Suomessa on käytössä ICD-10 tautiluokitus, joka määrittelee diagnoosin F64.0 Transsukupuolisuus (Transsexualism) seuraavasti:
A desire to live and be accepted as a member of the opposite sex, usually accompanied by a sense of discomfort with, or inappropriateness of, one’s anatomic sex, and a wish to have surgery and hormonal treatment to make one’s body as congruent as possible with one’s preferred sex.

ICD-10 lisämateriaalit määrittelee myös kyseiselle diagnoosille kliinisen kuvaksen ja diagnostiset ohjeistukset (Clinical Descriptions and Diagnostic Guidelines), jotka ovat:
A desire to live and be accepted as a member of the opposite sex, usually accompanied by a sense of discomfort with, or inappropriateness of, one’s anatomic sex and a wish to have hormonal treatment and surgery to make one’s body as congruent as possible with the preferred sex.
Diagnostic guidelines
For this diagnosis to be made, the transsexual identity should have been present persistently for at least 2 years, and must not be a symptom of another
mental disorder, such as schizophrenia, or associated with any intersex,
genetic, or sex chromosome abnormality.

Diagnoosille on myös olemassa diagnostiset kriteerit tutkimusta varten (Diagnostic Criteria for Research):
- Desire to live and be accepted as a member of the opposite sex, usually accompanied by the wish to make one’s body as congruent as possible with one’s preferred sex through surgery and hormonal treatment.
- Presence of the transsexual identity for at least two years persistently.
- Not a symptom of another mental disorder, such as schizophrenia, or associated with chromosome abnormality.
